Reasons to fill in the form:
- to start a new matter for you;
- to advise a change of address or other details;
- to correct some information that you know is incorrect;
- to let us know details of other people involved in your matter, or
- to make sure we know how to contact you.
You can fill in as many of these forms as are necessary. For example where you want to give us details of various beneficiaries, fill in a form for each one of them.
